CONTENT


The bill would amend the Public Health Code to provide that vital records and statistics forms or blanks, including those in an electronic format, could not be used unless they were provided or approved by the State Registrar. Currently, under the Code, a local registrar must furnish forms or blanks to persons required to file vital records and statistics. Only those forms or blanks provided by the State Registrar may be used.
