A bill to amend 1978 PA 368, entitled "Public health code," by amending section 2815 (MCL 333.2815), as amended by 1985 PA 20. THE PEOPLE OF THE STATE OF MICHIGAN ENACT: 1 Sec. 2815. (1) A county board of commissioners and the gov- 2 erning body of a city having a population of 40,000 or more may 3 agree that the county clerk or the clerk's deputy shall act as 4 the local registrar for the city. 5 (2) A local registrar shall DO ALL OF THE FOLLOWING: 6 (a) Record and transmit vital records and statistics as 7 required by this part. 8 (b) Furnish blank forms and instructions provided by the 9 state registrar to persons required to file vital records and 02418'97 CPD 2 1 vital statistics. A form or blank other than those provided OR 2 APPROVED by the state registrar shall not be used. 3 (c) Examine each vital record before accepting the record 4 for registration. If the record is incomplete or unsatisfactory, 5 the local registrar shall require the submission of additional 6 information necessary to complete the record before accepting it 7 for registration. 8 (d) Affix his or her identification to each vital record 9 accepted for registration and document the date of its 10 acceptance. 11 (e) Transmit, in the manner prescribed by the state regis- 12 trar, the vital record to the department. The local registrar 13 shall preserve at the local registrar's office information pre- 14 scribed by the state registrar. 15 (f) Issue a certificate of registration for a live birth on 16 a form approved by the state registrar and issue certified copies 17 of vital records documents on file pursuant to sections 2881, 18 2882, and 2891. 19 (g) Issue a permit for final disposition of a dead body upon 20 receipt of sufficient evidence that death occurred within the 21 local registrar's jurisdiction. 02418'97 Final page.
