A bill to amend 1978 PA 90, entitled
"Youth employment standards act,"
by amending section 5 (MCL 409.105).
THE PEOPLE OF THE STATE OF MICHIGAN ENACT:
1 Sec. 5. An issuing officer shall issue a work permit only
2 upon application in person by the minor desiring employment and
3 after having examined, approved, and filed the following
papers
4 DOCUMENTS:
5 (a) A statement of intention to employ, signed by the pro-
6 spective employer or by a person authorized by the prospective
7 employer, setting forth IDENTIFYING the general
nature of the
8 occupation in which the employer intends to employ the minor, the
9 hours during which the minor will be employed, the wages to be
10 paid TO THE MINOR, and other information the department of

1 education, in cooperation with the department of labor
CONSUMER
2 AND INDUSTRY SERVICES, requires.
3 (b) Evidence showing that the minor is of the age required
4 by this act. Proof of age shall be established by one
1 of the
5 following:
6 (i) A certified copy of the MINOR'S birth record or any
7 other proof of age showing the place and date of HIS OR HER
8 birth.
9 (ii) A certified copy of A valid MOTOR VEHICLE
operator's
10 license issued TO THE MINOR by this state clearly showing THE
11 MINOR'S date of birth.
12 (iii) The MINOR'S school record or the school census
13 record.
14 (iv) The sworn statement of the minor's parent or
guardian,
15 together with a corroborating statement of a physician.
16 (v) If documentary proof as described in subparagraphs
(i)
17 to (iv) is not obtainable, the issuing officer may accept
other
18 documentation as established by IN ACCORDANCE
WITH department
19 of education rules.
20 (C) VERIFICATION THAT THE MINOR HAS NOT HAD REPEATED, ERRAT-
21 IC, OR UNEXCUSED ABSENCES FROM SCHOOL DURING THE ACADEMIC YEAR
22 IMMEDIATELY PRECEDING THE MINOR'S APPLICATION FOR A WORK PERMIT.
